To explore whether a digital toolkit is useful for adults living with long-term health conditions who are also experiencing financial and/or social challenges. Findings will inform a future study tested in NHS settings.

People who are living with two or more long-term health conditions; are experiencing financial and/or social difficulties that affect their health or daily life; and are willing to use the digital toolkit over an 8-week period and provide feedback about their experiences.

Summary
Many people living with multiple long-term health conditions face additional challenges such as financial pressures, social isolation or difficulties accessing support. These challenges can make it harder to manage their health and wellbeing.
Working alongside people with lived experience, healthcare professionals and community organisations, researchers at University College London (UCL) have co-designed a digital self-management toolkit to provide practical information, resources and support. This study will explore how people use the toolkit, whether they find it helpful, and whether the study processes are suitable for a future larger evaluation in NHS settings.
Participants will use the toolkit for 8 weeks and be invited to share their experiences to help improve the resource for others. Participants would be compensated for their time for doing the questionnaires (up to £50).
